What to Do

Visit reception for:

• a registration form - you can complete this at the time or bring it back later.
• a practice booklet - this will tell you about our services.

• Registration Forms
  • for a new permanent registered patient => GMS 1
  • for a newborn baby's first registration => FP58
  • for a temporary resident => GMS 3

Once you have completed the registration form please hand it in to reception. We will then register you and you can then:

• make an appointment to see a nurse for a 'new patient' health check.
• make an appointment to see a doctor if you need a repeat prescription (please bring your old prescription with you when you come!) or if you have outstanding hospital appointments.
• register to use the internet for appointments and repeat prescriptions if you wish.
