Midwives
The Queen Elizabeth Hospital Iceni Midwifery Team is responsible for the maternity care of pregnant patients and liaises with the GPs as required. A midwife holds an ante-natal clinic in the surgery on Wednesday afternoons.
Health Visitor
Our health visitor Julia James operates form the Swaffham Community Hospital and usually works Monday to Wednesday. She is an experienced nurse with midwifery experience and additional special training. Her main concern is child care and development for children between 0-4 years of age, but also helps everyone within the family. She will discuss health and family problems and/or help you find the services you. Helping Julia in her child care work is Natasha Townsend, a community nursery nurse, who works Wednesday to Friday. Dieticians
The community dietician based at the Queen Elizabeth Hospital conducts a clinic every 4 weeks at the surgery on a Tuesday afternoon between 2.00-5.00pm She does not see patients who are just overweight; they should try to reduce weight through a “weightwatchers” or similar programme. GPs will refer patients who have more complex medical needs coupled with a dietary problem.
Family Action Service
Family Action, formerly the WellFamily service, a national charity, works closely with the Practice providing practical, emotional and financial support to individuals and families. Patients can be referred by their GP or refer themselves to Family Action. Telephone 01760 720302.
Community Mental Health Support Worker
The community mental health support worker is based at the Fermoy Unit at the Queen Elizabeth Hospital, but conducts limited outreach clinics at the surgery every fortnight for patients referred by their GP
Community Paramedic
The East Anglian Ambulance NHS Trust’s community paramedics operate in conjunction with the Swaffham practices. They support us by attending emergency calls for medical help, liaising with the doctors as appropriate. The men and women in green provide a fast response for accidents and emergencies without interrupting routine surgeries for other patients, but at the same time the close liaison with the duty doctors ensures that both the paramedic and the surgery know exactly what is going on.
